Duties:
Job Summary:
-The Social Worker provides direct social work services to students and families, assisting with personal and environmental issues surrounding social issues and lifestyle changes. Home care setting.
-Collaborates with students, families and the school staff to provide an effective plan to meet the pediatric/adolescent student needs during their school and occasionally.
-Provide social work services to pediatric/adolescent student populations through implementation of the five phases of the social work process.
-Provide referrals to community resources and agencies.
-Communicate effectively with students, families and school staff promoting patient satisfaction and the achievement of goals.
Essential Functions:
-Coordinate student care with other members of the school staff to ensure achievement of desired outcomes, quality, and efficient utilization of resources.

Required Skills & Experience:
-Knowledge about the care of the pediatric and adolescent patient.
-Previous home care experience.

-Completion of a Master's Degree in Social Work from an accredited program.
Preferred Education:

Required Certifications & Licensure:
-Current licensure as a Social Worker in the State of Michigan: Masters Social Worker license (LMSW) OR Limited Masters Social Worker License (LLMSW)
-Valid Michigan driver’s license and a dependable vehicle insured as required by the State of Michigan.
Preferred Certifications & Licensure:

13 week contract REQUIRED: 5+ years case management experience with 2+ years recent acute inpatient case management experience, Master of Social Work, LMSW licensure in the state of MI, Discharge planning experience. There will be limited training on site - candidates will need to be able to adapt quickly. Manager will be reaching out to candidates directly for interview - if your candidate is in "Client Review" status please make sure they are aware. MANAGER IS VERY STRICT ON THE RECENT INPATIENT CASE MANAGEMENT EXPERIENCE - TELEPHONIC, UTILIZATION REVIEW, PSYCHOLOGICAL EXPERIENCE DOES NOT APPLY TO THIS REQUIREMENT.

 

 

8 hour and 10 hour shifts available, no holidays, weekend rotation required. Later shift would be heavily supporting the ED. 8's: 8am-4:30pm or 3pm-11:30pm (ED) 10's: 3pm-1:30am (ED)
