SHIFT TIMES: 08:00:00 AM - 04:30:00 PM Monday-Friday 730am-4pm or 8am-4:30pm; Every 4th Weekend, rotating holidays

WORK SITE: 45 Reade Place, Poughkeepsie, NY 12601

Social Worker with Case Management experience in an acute care environment and in Transitions of Care/Discharge Planning.

Internal floating within the Case Management Team required to ensure coverage of all inpatient & outpatient coverage areas.

Discharge planning services such as:

• Skilled nursing facility placement

• Home care coordination

• Durable medical equipment arrangements

• Transportation coordination

• Insurance authorization support

• Family communication and discharge education

Required:

NYS LCSW or LMSW

3 years Acute Care Hospital Discharge Planning Experience

Cerner experience

WellSky experience
