Title: Licensed Clinical Social Worker

Duration: 13+ Weeks + High possibility of extension

Job Description:

Seeking compassionate LSWAIC-certified Case Managers for inpatient/acute care coordination, discharge planning, and utilization review. Epic-proficient professionals needed across dynamic acute care units.

 

Key Responsibilities

  • Coordinate inpatient care transitions, discharge planning, and community resource linkage for diverse patient populations.
  • Collaborate with MDs, RNs, and therapy teams to optimize LOS, ensure medical necessity, and execute safe handoffs.
  • Manage utilization review, payer authorizations, and compliance with CMS/InterQual criteria using Epic workflows.
