Job Description:

We are seeking an experienced RN Case Manager with a strong background in inpatient acute care settings. The RN Case Manager will be responsible for coordinating patient care, facilitating safe and timely discharges, and ensuring appropriate utilization of healthcare resources while collaborating with interdisciplinary teams.

Key Responsibilities

• Manage a caseload of inpatient patients across acute care units (Med-Surg, ICU, Telemetry, ED, etc.)
• Perform comprehensive patient assessments to identify clinical, psychosocial, and discharge planning needs
• Coordinate safe and timely discharge planning, including placement to SNF, rehab, LTAC, or home care
• Collaborate with physicians, nurses, social workers, and ancillary departments to ensure continuity of care
• Conduct utilization review (UR) and apply InterQual / MCG criteria to determine medical necessity and level of care
• Manage payer communication, including authorizations, concurrent reviews, and denials
• Facilitate care transitions and coordinate services such as DME, home health, infusion, and hospice
• Educate patients and families regarding treatment plans, post-discharge care, and available resources
• Maintain accurate and timely documentation in EMR systems (e.g., EPIC, Cerner)
• Ensure compliance with hospital policies, regulatory standards, and CMS guidelines

Required Qualifications

• Active CA Registered Nurse license
• 3+ years of RN Case Management experience in an acute care / inpatient hospital setting
• Strong experience with discharge planning and care coordination
• Knowledge of InterQual and/or MCG guidelines
• Experience with utilization review and payer communication
• Familiarity with EMR systems (EPIC, Cerner, or equivalent)
• Excellent communication and interdisciplinary collaboration skills
