Unit Overview
- Beds/Residents: 120
- EMR: My Unity
- Patient Population:
- Moderate to severe Alzheimer’s
- Deconditioned rehab patients
- 5 comfort care beds (travelers typically do not staff these)
Staffing & Ratios
- Nurses (RN/LPN):
- Current: Days 1:38 | Nights 1:38 (low acuity)
- Preferred: 2 nurses per “Neighborhood” (~1:19)
- Medication Aides:
- 1 on AM shift
- 1 on PM shift
- CNAs:
- Days: 1:9 (goal 1:6–8; typically 5–6 CNAs per neighborhood)
- Nights: 1:19
- Note:
- Generally well-staffed, currently impacted by COVID outbreak shortages
Responsibilities & Workflow
- Admissions/Discharges:
- ~14–17 admissions/month
- Discharges are rare
- Daily Duties:
- Rounding & documentation
- Administer medications (including narcotics – not given by med aides)
- Diabetic assessments
- Fall assessments
- Monthly assessments
- Supervise/manage CNAs
- Assist with meals (high dysphagia/Alzheimer’s population)
- Treatments: Rare
Support & Team Structure
- Charge Nurse / Team Lead each shift
- DON/Managers available on call
- Physician on call after hours
- Interdisciplinary Team:
- PT / OT / SLP
- Case Managers
- Social Workers
- Dieticians
Respiratory & Pharmacy
- Respiratory Therapy: Not on-site
- Nurses handle treatments
- Rare nebulizers
- Occasional inhalers (MDI)
- Pharmacy: Off-site
Scheduling & Shifts
- Shift Times: 12-hour shifts (6:45 AM – 7:15 PM / PM equivalent)
- Scheduling:
