Description

Job Title: LCSW – Inpatient Case Management

Shift: Day (5x8 Hours, 08:00 AM – 04:30 PM)

Job Summary

We are seeking a Licensed Clinical Social Worker (LCSW) for an inpatient case management role. The candidate will be responsible for coordinating patient care, discharge planning, and supporting patients with social, emotional, and financial needs in an acute care setting.

Key Responsibilities

Perform biopsychosocial assessments
Develop and implement patient-centered care plans
Coordinate discharge planning and post-acute services
Collaborate with interdisciplinary teams for care coordination
Assist patients with community resources (Medicaid, housing, transportation, etc.)
Provide crisis intervention and counseling support
Ensure accurate documentation using Epic EMR system

Requirements

Active LCSW / LSWAIC / MSW license
Minimum 1 year inpatient or acute care experience
Epic charting experience required
Certifications: BLS required
Strong knowledge of case management & care coordination
