Shift: Evening Shift (12:00 PM – 8:30 PM)

Job Summary

The Hospital-Based Social Worker provides psychosocial support, counseling, and discharge planning services to patients and their families. The role involves collaborating with the healthcare team to address social, emotional, and environmental factors that affect patient health and recovery.

Roles and Responsibilities

• Conduct psychosocial assessments to evaluate patients' mental, emotional, and social needs.
• Provide counseling and crisis intervention for patients and families.
• Develop and implement discharge plans in collaboration with physicians, nurses, and care teams.
• Coordinate community resources and support services for patients after discharge.
• Assist patients and families with coping strategies, adjustment to illness, and healthcare decisions.
• Facilitate communication between patients, families, and healthcare providers.
• Advocate for patients to ensure access to appropriate care, services, and resources.
• Maintain accurate documentation of assessments, care plans, and interventions in the medical record.
• Ensure compliance with hospital policies, ethical standards, and regulatory requirements.
• Participate in multidisciplinary care team meetings and case management discussions.

Minimum Requirements

• Minimum 2 years of experience within the social work in Hospital specialty
• Hospital-based experience required
• LCSW (Licensed Clinical Social Worker) license in the State of Georgia required
• AHA BLS (Basic Life Support) certification required
