Licensed Practical Nurse (LPN) – Long Term Care (Permanent Placement)

Job Summary

A large care facility with 212 resident rooms, offering long-term care, short-term rehabilitation, and memory care services, is seeking dedicated Licensed Practical Nurses (LPNs) for permanent placement. This role is ideal for local nurses who are passionate about providing compassionate, resident-centered care in a supportive environment.

Key Responsibilities

• Provide direct nursing care and support under the guidance of Registered Nurses and clinical leadership.
• Assist residents in long-term care, short-term rehab, and memory care settings with daily medical needs.
• Administer medications and treatments as ordered and in accordance with facility policies.
• Monitor resident conditions and promptly report changes to the supervising nurse or provider.
• Document care, observations, and interventions accurately and in a timely manner.
• Support interdisciplinary collaboration to ensure continuity and quality of care.
• Maintain infection control standards, safety protocols, and regulatory compliance.
• Communicate effectively and professionally with residents, families, and staff.

Requirements

• Active LPN license in the state.
• Current CPR and First Aid certifications (required).
• Experience in long-term care, skilled nursing, or rehabilitation preferred.
• Strong communication, observation, and clinical skills.
• Ability to work in a team-oriented, fast-paced care environment.
• Local candidates only; no travel positions.
