Licensed Practical Nurse (LPN) – Rehabilitation (Permanent Placement)

Job Summary

A large, multi-level care facility with 212 resident rooms, offering long-term care, short-term rehabilitation, and memory care services, is seeking skilled Licensed Practical Nurses (LPNs) for permanent placement within the Rehabilitation Unit. This role is ideal for local LPNs who are passionate about supporting patients through recovery and regaining independence.

Key Responsibilities

• Provide direct nursing care to residents undergoing rehabilitation following illness, injury, or surgery.
• Assist with assessments, implement care plans, and deliver nursing interventions based on resident rehabilitation needs.
• Administer medications and treatments as ordered by providers and in accordance with facility procedures.
• Monitor resident progress, document changes, and report updates to supervising RNs and interdisciplinary team members.
• Support therapy goals by collaborating with physical, occupational, and speech therapy staff.
• Maintain accurate and timely documentation of all care provided.
• Uphold safety, infection control, and regulatory standards.
• Promote a positive, encouraging environment that supports rehabilitation outcomes.
• Communicate professionally with residents, families, and team members.

Requirements

• Active LPN license in the state.
• Current CPR and First Aid certifications (required).
• Experience in rehabilitation, post-acute care, or skilled nursing preferred.
• Strong observation, clinical, and communication skills.
• Ability to work effectively in a team-oriented, fast-paced rehab setting.
• Local candidates only (no travel positions).
