Description

Physical Therapist – Home Health

Location: Renton / South Seattle (South King County)
Home Base: Renton 9
Contract: 13 weeks
Start: Apr 27, 2026

Schedule

Hours: 36 hrs/week (guaranteed)
Shift: Mon–Fri | 8:00a–4:30p
Call/Weekends: None
Holidays: Potential availability as needed

Setting

Home Health
Coverage throughout South King County
Mileage reimbursed between visits

Productivity

Expectation: 30 points/week

Responsibilities

Perform PT evaluations and develop plans of care per CMS and WA guidelines
Deliver skilled PT services using evidence‑based practice
Coordinate care with physicians, nursing, and interdisciplinary team
Monitor and report patient response and changes in condition
Complete visit notes, progress notes, and discharge summaries on time
Educate patients, caregivers, and care team on goals and limitations
Supervise PTA and home health aides as allowed by state regulation
Participate in staff meetings, in‑services, and agency initiatives

Requirements

Active WA PT license or compact privilege
BLS certification
Valid driver’s license, auto insurance, reliable vehicle
Ability to travel across service area

Mileage

72.5¢ per mile (federal rate)
