Description

Home Health / Hospice RN

Shift: Days, 8:00 AM – 4:30 PM, Monday–Friday
Guaranteed Hours: 40/week
Contract: 13 weeks

Requirements:

Experience: Travel experience preferred; must have skills in chronic disease management, NPWT, ostomy care, central line care, IV administration, complex wound care, hospice care (pain & symptom management, end-of-life care)
Education: RN license required; BSN not required
Certifications: BLS (AHA)

Job Details:

Care for primarily geriatric home health & hospice patients (CHF, COPD, cancer, diabetes, stroke, dementia, wounds, post-surgical care)
Daily patient visits: 5–6/day, 25–30 points/week
Perform routine & OASIS visits, patient/family education, and symptom management
Attend daily huddle, weekly IDT meeting, monthly staff meeting
On-call: 1 week every 6 weeks; must cover at least 1 major and 1 minor holiday
No floating required, but may work both home health & hospice visits
Charting: Cerner
Dress code: Solid scrubs, closed-toe non-slip shoes
