Description
Job Title: LCSW – Inpatient Case Management
Location: Seattle, WA
Job Overview:
We are seeking a Licensed Clinical Social Worker (LCSW) for a 13-week contract in a fast-paced acute care setting. This role focuses on inpatient case management, care coordination, and discharge planning for neuro and cardiac patients at a comprehensive stroke center. First-time travelers are welcome!
Schedule:
- Shift: Days (8:00 AM – 4:30 PM)
- Schedule: 5x8s
- Weekend Coverage: Required (minimum 2 days/month)
Responsibilities:
- Perform biopsychosocial assessments and develop care plans
- Coordinate discharge planning and post-acute care services
- Collaborate with interdisciplinary teams
- Provide crisis intervention and patient/family support
- Connect patients with community resources and services
- Document care in Epic system
Requirements:
- Minimum 1 year of inpatient/acute care case management experience
- LCSW or LSWAIC certification required
- BLS certification
- Epic experience required
