Social Worker for the Rehab Unit
Start date: ASAP
13 weeks
• Shift Time: 830a-5p
• Days: M-F
• Experience required: at least 1 year of hospital case management (discharge planning) experience required. Knowledge of Cerner.
• Certifications required: LICSW, active DC SW license
" Fulfills the needs of patients, families and community by providing Social Work services in accordance with his/her licensure, certification, training, and regulatory requirements. Assumes the responsibility for coordinating and facilitating services for patients during and subsequent to admission to the acute rehabilitation unit. Conducts discharge planning, coordination of care, and other planning and counseling activities to effectively move the patient through the continuum of care in a cost-effective and resource efficient manner. Responds effectively to new and changing work assignments."
