Description

Home Health – Physical Therapist - Days

Start Dates: ASAP

13-week contract
40 hours per week
Monday-Friday
Hours: 8-5:30 pm (visits usually won’t start until 9-10a)
Shift: Days
Certs: BLS
License: CA PT
Experience:
Must have 1 year of home health experience
HCHB: not a must, will provide training for this
Oasis: not a deal breaker, SOC’s are done by the RNs and staff OT/PTs
Productivity expectations: 6 patients a day, they will load them with patients the night before, and they will make calls to schedule the appointments
