<p><strong>Home Health – Physical Therapist - Days</strong><br /> </p><p><strong>Start Dates: ASAP</strong></p><ul><li>13-week contract</li><li>40 hours per week</li><li>Monday-Friday</li><li>Hours: 8-5:30 pm (visits usually won’t start until 9-10a)</li><li>Shift: Days</li><li>Certs: BLS</li><li>License: CA PT</li><li>Experience:</li><li>Must have 1 year of home health experience</li><li>HCHB: not a must, will provide training for this</li><li>Oasis: not a deal breaker, SOC’s are done by the RNs and staff OT/PTs</li><li><strong>Productivity expectations: </strong>6 patients a day, they will load them with patients the night before, and they will make calls to schedule the appointments</li><li>Geriatric population</li></ul>
