Description
Pride Health is hiring a Physical Therapist client in the medical facility. This is a temporary assignment opportunity and a great way to start working with a top-tier healthcare organization!
Job Type: Contract
Location-Charleston, WV
Shift: 8 am to 4.30 pm
Start Date: 04/20/2026
Travel: Coverage Areas: Kanawha County
Mileage Reimbursement: Yes
Specific Duties:
· Evaluate patients’ functional status and physical therapy needs in compliance with CMS guidelines.
· Develop and implement individualized treatment plans in collaboration with physicians.
· Deliver evidence-based therapy to relieve pain, restore function, and improve mobility.
· Monitor and document patient progress, reporting changes to physicians and nurse supervisors.
· Educate patients, families, and caregivers on treatment plans, progress, and functional limitations.
· Complete timely and accurate documentation, including evaluations, progress notes, and discharge summaries.
· Travel to patient homes within the assigned service area and nearby locations as needed.
· Participate in care coordination, team meetings, and case conferences.
· Supervise Physical Therapist Assistants (PTAs) and Home Health Aides as applicable.
· Maintain compliance with agency policies, state regulations, and professional standards of practice.
· Participate in continuing education to maintain licensure and clinical competency.
Qualifications:
· Active Physical Therapist license
· Current CPR certification.
· Valid driver’s license, auto insurance, and reliable transportation.
· Experience in home health or related clinical settings preferred.
