Job Title - Home Health Physical Therapist

Job Type: Contract

Location- Fort Morgan, CO (50-mile coverage area)

Shift:  Monday–Friday 8:00 am to 4:30 pm/ 40 hrs/ week 
Pay - $2755.40/ week

Mileage reimbursement - Yes 
 

Job Summary:
We are seeking a skilled and reliable Travel Physical Therapist to join our team. This role is focused on performing high-volume blood draws in a clinical setting while ensuring patient comfort, safety, and accurate specimen collection.

Key Responsibilities:

• Perform in-home patient evaluations & develop care plans
• Deliver skilled PT services (mobility, balance, pain mgmt)
• Improve patient independence & safety
• Complete OASIS documentation & visit notes
• Collaborate with interdisciplinary care teams 

Required Qualifications:

• MPT or DPT
• Home Health OASIS experience
• Strong clinical & documentation skills
• Ability to travel within the assigned area 

Preferred Experience:

• HCHB (Homecare Homebase) experience
