Pride Health is hiring a Physical Therapist to support our clients in the medical facility. This is a temporary assignment opportunity and a great way to start working with a top-tier healthcare organization!
 

Job Title - Home Health Physical Therapist

Job Type: Contract

Location- Flossmoor IL and Surrounding Areas

Shift:  Monday–Friday 8:00 am to 4:30 pm (40 hrs/ week)
Pay - $54 -$56/ hr on W2 
 

Job Summary:
We are seeking a skilled and reliable Travel Physical Therapist to join our team. This role is focused on performing high-volume blood draws in a clinical setting while ensuring patient comfort, safety, and accurate specimen collection.

Key Responsibilities:

  • Conduct comprehensive patient evaluations
  • Develop & implement individualized care plans
  • Provide skilled PT treatments (mobility, balance, pain management )
  • Complete OASIS & clinical documentation accurately
  • Educate patients & caregivers on safety & exercises
  • Collaborate with interdisciplinary care teams
  • Monitor progress & adjust treatment plans
  • Ensure compliance with Medicare & regulatory standards
  • Travel within assigned coverage area 

Required Qualifications:

  • Active PT License (IL)
  • CPR/BLS Certification
  • 1+ year clinical experience (Home Health preferred)
  • Knowledge of Medicare guidelines & documentation
  • Strong clinical, communication & decision-making skills
  • Valid driver’s license & reliable transportation

Preferred Experience:

  • Home health or community-based care experience
  • OASIS documentation experience
  • EMR/point-of-care documentation skills
  • Geriatrics & chronic disease management experience
  • Bilingual skills
