ER RN (Critical Access / Cross-Trained)
Location: Pahala, HI (Big Island)
Contract Duration: 13 Weeks (Starts 05/11/2026)
Shift: 12-Hour Shifts | 72 Hours per Pay Period (6x12s every 2 weeks)
Schedule: Every 3rd weekend off
Job Summary
A unique Critical Access Hospital located in a remote area of Hawaii's Big Island. This is a "true" rural nursing role: because the facility is small, there are no separate units. You will be hired as an ER RN, but you must be a clinical "generalist" capable of covering Emergency, Acute Care, and Long-Term Care (LTC) patients simultaneously. This role requires extreme flexibility, high autonomy, and the ability to pivot from a trauma stabilization to a routine LTC medication pass in a single shift.
Key Responsibilities
- Emergency Response: Act as the primary lead for any ER presentations; stabilize, treat, and coordinate transfers for higher-acuity trauma or cardiac cases.
- Acute & LTC Coverage: When the ER is quiet, you will provide backup and supplemental staffing for the acute care and LTC wings.
- Medication Administration: Perform comprehensive med passes and nursing procedures for both acute and long-term care residents.
- Cross-Continuum Care: Manage a diverse patient load ranging from acute medical emergencies to chronic geriatric care.
- Resource Management: Operate effectively in a facility with limited onsite resources; utilize Epic EMR for all documentation.
Requirements & Qualifications
- Experience: Minimum 2 years of Emergency Department (ED) experience is mandatory.
- Licensure: Must hold an active State of Hawaii (HI) RN License by the start date (05/11/2026). If not currently held, you must apply immediately upon offer acceptance.
- Certifications: AHA BLS (Required). Given the ER focus, ACLS and PALS are highly recommended for rural autonomy.
- Adaptability: Must be comfortable working in a non-latex-free facility and rotating through all areas of the hospital.
Unit & Facility Details
- Location: Pahala, HI 96777.
- Bed Size: Small (0–49 beds).
- Environment: Rural, remote, and critical access.
- Travel Note: You must arrive the Thursday before your start date (May 7th) to attend a mandatory Employee Health appointment on Friday.
Financial & Logistics Notes
- Guaranteed Hours: Hours are NOT guaranteed for this contract; however, the schedule is set for 72 hours per pay period.
- Holiday Coverage: Required as part of the rotation.
- Onboarding: Prompt compliance is essential for HI licensing and health screening.
