Job Summary

Seeking a professional Outpatient RN for their Internal Medicine clinic. This role is ideal for a nurse who excels in primary care coordination, chronic disease management, and patient triage. Operating in a premier island community, you will serve as a vital link between patients and providers, ensuring high-quality longitudinal care for the year-round resident population and seasonal visitors.

Key Responsibilities

• Clinical Triage: Perform phone and in-person triage; assess patient symptoms and determine the appropriate level of urgency for provider intervention.
• Chronic Disease Management: Assist in the management of complex adult patients, including those with hypertension, diabetes, and COPD.
• Procedural Support: Assist providers with minor office procedures, wound care, and medication administration (injections/vaccinations).
• Care Coordination: Manage laboratory results, medication refills, and specialty referrals; facilitate patient education regarding new diagnoses or treatment plans.
• Patient Education: Provide comprehensive teaching on self-care, medication adherence, and lifestyle modifications to improve health outcomes.

Requirements & Qualifications

• Experience: Primary care and/or outpatient experience is preferred. 2+ years of clinical nursing experience is standard.
• Licensure: Active Massachusetts (MA) RN License.
• Certifications: AHA BLS (Required).
• Compliance: Current Flu Vaccine is required.
• Logistics: A personal car is required for this assignment to navigate the island and reach the hospital.

• Location: Oak Bluffs, MA 02557.
• Environment: Outpatient clinic setting within a Critical Access Hospital.
• Start Date: 05/04; confirm the final date with your recruiter.
