Description
Position: Licensed Master Social Worker (LMSW)
Location: New York, NY
Expected Hours: 7 hours per day | 35 hours per week
Hospital experience is Required!!
Job Overview:
The Licensed Master Social Worker (LMSW) will provide inpatient medical social work services within a hospital setting, supporting patients admitted to medical and surgical units. This role focuses on psychosocial assessment, discharge planning, and care coordination to ensure safe and effective transitions of care.
Key Responsibilities:
- Conduct psychosocial assessments for hospitalized patients and evaluate social, emotional, and environmental needs.
- Coordinate discharge planning for patients transitioning to home care, rehabilitation facilities, skilled nursing facilities, or other community resources.
- Collaborate with interdisciplinary hospital teams including physicians, nurses, case managers, and therapists to develop appropriate care plans.
Facilitate patient and family meetings to discuss discharge plans, resources, and care coordination.
Required Qualifications:
- Master’s Degree in Social Work (MSW) from an accredited program.
- Active Licensed Master Social Worker (LMSW) license issued by the New York State Education Department (NYSED).
- Minimum 1 year of social work experience,
- Inpatient medicine, surgery, med/surg experience required.
