Job Description: Licensed Practical Nurse (LPN) – Long Term Care (Contract)

Shift: Variable Shifts (based on census and staffing needs)

Position Overview

We are seeking an experienced Licensed Practical Nurse (LPN) for a 13-week contract assignment in a Long Term Care (LTC) setting. The ideal candidate must be proficient in providing comprehensive nursing care to residents while collaborating with a multidisciplinary team to support daily operations and promote quality outcomes.

Key Responsibilities

  • Perform resident assessments and implement appropriate care plans.
  • Administer medications and treatments in accordance with facility protocols.
  • Notify physicians or nurse practitioners of significant changes in resident condition.
  • Assist with admissions, discharges, and transfers.
  • Provide ADL (Activities of Daily Living) support as needed.
  • Perform gastrostomy tube care, urinary catheter care, and nebulizer treatments.
  • Manage wound care, including wound vac therapy.
  • Provide CPAP support and respiratory-related interventions.
  • Maintain accurate documentation in the facility’s EMR system.
  • Support additional clinical tasks as assigned.

Requirements

  • Active LPN License (state license must be obtained by acceptance or prior to assignment start).
  • Minimum 2 years of recent LTC experience required.
  • BLS/BCLS certification required.
  • Must complete employee health screening prior to start (including flu vaccination or approved exemption).
  • Must be able to arrive one day before onboarding for health clearance.
  • Ability to work various shifts based on facility census and needs.
  • Holiday coverage required as assigned.
