Position Overview

We are seeking an experienced Licensed Practical Nurse (LPN) for a 13-week contract assignment in a Long Term Care (LTC) setting. The ideal candidate must be proficient in providing comprehensive nursing care to residents while collaborating with a multidisciplinary team to support daily operations and promote quality outcomes.

Key Responsibilities

Perform resident assessments and implement appropriate care plans.
Administer medications and treatments in accordance with facility protocols.
Notify physicians or nurse practitioners of significant changes in resident condition.
Assist with admissions, discharges, and transfers.
Provide ADL (Activities of Daily Living) support as needed.
Perform gastrostomy tube care, urinary catheter care, and nebulizer treatments.
Manage wound care, including wound vac therapy.
Provide CPAP support and respiratory-related interventions.
Maintain accurate documentation in the facility’s EMR system.
Support additional clinical tasks as assigned.

Requirements

Active LPN License (state license must be obtained by acceptance or prior to assignment start).
Minimum 2 years of recent LTC experience required.
BLS/BCLS certification required.
Must complete employee health screening prior to start (including flu vaccination or approved exemption).
Must be able to arrive one day before onboarding for health clearance.
Ability to work various shifts based on facility census and needs.
Holiday coverage required as assigned.
