Job Title: Registered Nurse (RN) – Community Living Center (CLC)

Location: Cleveland VA Medical Center, 10701 East Blvd, Cleveland, OH 44106
Duration: Base Year + 1 Optional Year
Start: Pending Credentialing (6–8 weeks)
Positions: Multiple (Part of 45 FTEs)
Work Setting: Community Living Center (Long-Term Care / Rehab)

Job Summary

We are seeking experienced Registered Nurses (RNs) to support the Louis Stokes Cleveland VA Medical Center as part of a large-scale staffing initiative tied to the Federal Electronic Health Record (EHR) implementation. RNs will play a critical role in delivering high-quality patient care to veterans in the Community Living Center (CLC), focusing on long-term, rehabilitative, and geriatric care.

Key Responsibilities

Provide direct patient care to veterans in a long-term care and rehabilitation setting
Assess, plan, implement, and evaluate individualized patient care plans
Administer medications and treatments in accordance with physician orders
Monitor patient conditions and document care accurately in the EHR system
Collaborate with interdisciplinary teams to ensure continuity of care
Educate patients and families on health conditions and care plans
Maintain compliance with VA policies, HIPAA, and regulatory standards
Support patient care during EHR system transition and implementation

Required Qualifications

Graduate of an accredited nursing program (ACEN or CCNE)
Active RN license (Ohio or Compact State - NLC)
Minimum 2+ years of recent direct patient care experience (within last 3 years)
Current BLS (Basic Life Support) certification (AHA required)
Strong clinical, communication, and documentation skills
Ability to work in a fast-paced, team-oriented environment

Preferred Qualifications

Experience in Community Living Center (CLC), Long-Term Care, or Skilled Nursing Facility
Prior VA or federal healthcare experience
Experience working with geriatric or rehabilitation patients
Familiarity with EHR systems
