Description
Job Title: Integrated Case Manager – Population Health
Location: Detroit, MI 48202
Department: Administration and Support – HFH Clinical Allied
Hours: 40 hours/week, 8 hours/day
Key Responsibilities:
- Conduct comprehensive assessments of patients and families, including medical, psychological, social, and functional needs.
- Develop personalized care plans to optimize patient outcomes and quality of care.
- Advocate for patients and families, providing education, guidance, and access to resources and services.
- Utilize professional judgment, critical thinking, motivational interviewing, and self-management techniques to support patient goals.
- Provide counseling and guidance on treatment decisions, including end-of-life care and Advanced Care Planning.
- Coordinate smooth transitions between care settings to ensure continuity and safety.
- Collaborate with interdisciplinary teams to co-manage patients with complex needs; facilitate care conferences as necessary.
- Evaluate patient response to care plans and adjust interventions as needed.
- Participate in process improvement initiatives to reduce risk and improve outcomes.
- Address sensitive cases, including child abuse, domestic violence, elder abuse, institutional abuse, and sexual assault.
Required Qualifications:
- Education: BSN (Nursing) OR MSW (Social Work)
- Licensure: RN (Registered Nurse) OR LMSW (Licensed Master Social Worker) with a valid, unrestricted Michigan license.
- Experience:
- Minimum 5 years of case management experience.
- At least 2 years of recent acute inpatient case management experience (telephonic or utilization review does not qualify).
- Strong verbal, written, and interpersonal communication skills.
- Knowledge of discharge planning, home health care, community health, Medicare/Medicaid regulations, case management principles, and social determinants of health.
- Proficient in computer systems and electronic health records.
- Excellent organizational, analytical, and problem-solving skills.
- Compassionate, with a strong patient advocacy mindset.
Preferred Skills:
- Experience facilitating community resources for diverse populations.
- Knowledge of preventive service guidelines and clinical practice guidelines.
