Description
Registered Nurse (RN) – Case Management (Day Shift)
Location: Lodi, CA
Department: Case Management
Contract Length: 13 Weeks
Schedule: Day Shift | 8:00 AM – 6:30 PM | 10‑hour shifts
Days Worked: Variable, may include weekends
Key Responsibilities
- Coordinate patient care across the continuum from admission to discharge
- Perform utilization review and assess medical necessity
- Collaborate with physicians, nursing, social work, and ancillary services
- Facilitate discharge planning including post‑acute services, DME, and placement
- Communicate with payers regarding authorizations and continued stays
- Educate patients and families on care plans and discharge needs
- Document care coordination activities accurately in Cerner
- Ensure compliance with regulatory and hospital standards
Required Qualifications
- Active RN License (state of practice)
- Minimum 1 year acute‑care nursing experience (required)
Cerner experience – REQUIRED
