Description

We are urgently seeking an experienced Home Health / Hospice Registered Nurse (RN) for a Rapid Response assignment. This role focuses on delivering skilled nursing care, case management, and coordination of services in patients’ homes or inpatient hospice settings.

Ideal for nurses experienced in independent fieldwork, patient education, and interdisciplinary care coordination.

🩺 Core Responsibilities

Provide skilled nursing care in home and hospice settings
Conduct patient assessments, care planning, and interventions
Manage case coordination and continuity of care
Supervise CNAs and Home Health Aides
Deliver patient and family education
Maintain accurate documentation (OASIS compliance)
Participate in on-call rotation and urgent patient needs response

🧠 Required Skills & Experience

Minimum 2+ years RN experience
Experience in Home Health, Hospice, or Acute Care
Strong skills in:
- Venipuncture
- Wound care
- Catheterization
- IV / parenteral therapy
Ability to work independently in field-based settings

✅ Requirements

Active Montana RN License
Valid Driver’s License + Auto Insurance
Reliable transportation (required for home visits)
