Shift Requirements: M-F 8-530
Float Requirements: occasional travel to central/ Northern regions
Mileage reimbursed at government approved rate
On Call/Call-Back: N/A
Weekend/Holiday: PT's rotate one Saturday every 6-8 weeks. (the Sat. they work would be given a day off during the week) Saturday only. No holiday.

Required Certifications: AHA or ARC BLS, Licensure required for the State of Wisconsin. NPI # required. If the candidate does not have one, they need to start the process of applying for a NPI # in order to start.
Required Education: Master's Degree in Physical Therapy.
Minimum Experience: Home Health experience, training working with the adult/geriatric population with a variety of orthopedic, neurological and functional mobility diagnosis. Knowledge of Physical Therapy standards and practices acquired through a minimum of a Masters degree in Physical Therapy.
Charting/Equipment: Laptop for point of service documentation, NetSmart EMR, and cell phone for GPS and communication.
Unit Specific Skillset/Unit Overview: Evaluates, treats and plans patient care in accordance with Physical Therapy Standards of practice. Population served includes adult and geriatric patients. Documents to meet regulatory and reimbursement requirements. Meets productivity expectations of an average of 4.5 visits per day
