Job title: Home Health Physical Therapist
Schedule: Mon-Fri 8am-4:30pm
Location: Orlando, FL 32805
Job Type: Local contract
Contract Length: 3+ months
 
*Pay offered is based on experience, expertise, credentialing, and education.  
 
Travel: Coverage Areas: ORANGE COUNTY mostly for east and central Orlando
Mileage Reimbursement: Yes

Responsibilities: 

  • Conduct comprehensive in-home physical therapy evaluations, assessing mobility, strength, balance, pain, endurance, and home safety
  • Develop, implement, and regularly update individualized Plans of Care (POC) based on physician orders, patient goals, and measurable outcomes
  • Deliver skilled physical therapy interventions to improve mobility, gait, balance, strength, endurance, and pain management
  • Educate patients and caregivers on diagnosis, safety precautions, fall prevention, home exercise programs, and use of medical equipment
  • Perform home safety assessments and recommend modifications to enhance safety and support independent functioning
  • Maintain accurate, timely, and compliant documentation within the electronic medical record (EMR)
  • Collaborate with interdisciplinary team members to coordinate care and optimize patient outcomes
  • Communicate changes in patient condition or barriers to care promptly to physicians and relevant stakeholders
  • Ensure compliance with payer requirements, including documentation standards and eligibility criteria
  • Manage caseload efficiently while meeting scheduling, productivity, and visit timeliness expectations

Must have qualifications/experience

  • HCHB AND OASIS proficient
  • Florida PT license
  • Current CPR certification.
  • Valid driver’s license, auto insurance, and reliable transportation.
  • Experience in home health or related clinical settings preferred.
