Description
Job Opportunity: OR Registered Nurse (RN)
Location: Waterloo, Iowa
Employment Type: Long-Term Assignment (LTA)
Contract Length: 13 weeks
Work Schedule
- 12-hour shifts: 6:00 AM – 6:30 PM
- 9-hour shifts: 9:00 AM – 6:30 PM
- 10-hour shifts: 8:00 AM – 6:30 PM
- Holidays included in call rotation
- Overnight call approximately every 1-2 weeks
- Weekend call approximately every 4-5th weekend
- Holiday call included in call rotation
- Block schedules are not guaranteed
Required Experience
- Specialty: OR Registered Nurse (RN)
- Years of Experience: Not explicitly stated; assume standard RN experience for OR setting (confirm with recruiter if needed)

Required Certifications and Licenses
- BLS (American Heart Association certification required for all direct patient care personnel)
- ACLS
- Valid RN License (must be verified within 30 days of start)

Health and Immunization Requirements
- Fit Test
- Trinity 10 Panel Drug Screen (9 or 10 panel, completed within 30 days of start, no THC)
- COVID-19 Vaccine (proof of booster by Sept 2025, or declination/exemption except CA or IL)
- MMR (proof of 2 vaccinations or IgG titer)
- Hepatitis B vaccination series (at least first vaccination prior to start)
- Influenza Vaccine (proof or declination/exemption, flu season 10/1/2024–4/30/2025)
- 2 Step PPD OR IGRA (Quantiferon/Tspot) with chest x-ray if positive TB as per regional requirements
- Varicella (proof of vaccination or IgG titer)
- Physical/Health Statement (exam within last year confirming fitness to work)
- Tdap vaccine (current, non-expired; declinations accepted if no pediatric interaction)
Professional and Compliance Requirements
- Signed Trinity Code of Conduct Form
- Trinity I9 form
- Confidentiality and System Security Agreement
- Integrity and Audit Services Form
- Non-Employee Upload Form (pages 1-3 and 28)
- Conflict of Interest Disclosure Form
- Criminal Background Check (Federal, Nationwide, County for last 7 years, including SSN verification, dated within 30 days of start)
- Education Verification (highest academic degree or diploma/GED)
- EPLS (Excluded Parties List System) check dated within 30 days
- GSA/OIG Exclusion check dated within 30 days
- Joint Commission CORE Exam within the last year
- NSO (National Sex Offender) check dated within 30 days
- System Access Form (completed and uploaded one week before start)
- Skills Checklist (provided by agency for clinical contractors)
- Child and Adult Dependent Abuse Training (Iowa only)
- Iowa Sing Report (Criminal History, Sex Offender, Dependent Adult & Child Abuse Registries check, within 30 days prior to start)
- OFAC Sanctions Screening (within 30 days of start)
This position requires a highly compliant and credentialed OR RN prepared for a long-term assignment with rotating call responsibilities including holidays and weekends.
