Job Title: Licensed Practical Nurse (LPN)
Facility: Serenity Spring at Scandia – Continuum Healthcare, Inc
Location: Sister Bay, Wisconsin (WI)
Shift: 8-Hour Day Shift (6:00 AM – 2:30 PM)
Weekly Hours: 40 hours
Pay Rate: $53.00/hour

Job Summary:

We are seeking experienced and compassionate Licensed Practical Nurses (LPNs) to join our team at Serenity Spring at Scandia. The LPN will provide quality nursing care to residents in a long-term care setting, ensuring their comfort, safety, and well-being during day shifts.

Key Responsibilities:

• Deliver direct patient care in accordance with individualized care plans
• Administer medications and treatments as prescribed
• Monitor residents’ conditions and report any changes to the healthcare team
• Assist residents with daily living activities as needed
• Maintain accurate documentation and medical records
• Collaborate with CNAs and other healthcare staff to ensure quality care

Schedule Details:

• Day Shift: 6:00 AM – 2:30 PM
• 40 hours per week

Additional Information:

• Experience in long-term care / nursing home setting preferred
• Must be reliable and able to work consistent day shifts

Requirements:

• Active LPN license
• Strong clinical and patient care skills
• Excellent communication and teamwork abilities
• Ability to provide compassionate, resident-centered care
