Description
Client Offer- LOCALS APPROVED AT SAME RATE
Required skills: Dysphasia Management, Patient Safety, Aural Rehabilitation, Functional Maintenance Programs, In-service Training, Patient/Family Education, Evaluation Tools (BIMS, Depression Screening, Oral Peripheral Exam), Billing/Documentation
Must have home health experience; OASIS training preferred
Certs required: BLS, CCC-SLP, state licensed
Scrubs: Any solid color scrub
Unit Info (share with your traveler): We cover Manatee County and South Hillsborough territories so must be willing to over multiple areas
Patients: Acute/chronic adolescent through geriatric
Skilled Home Care (Medicare, Medicare Advantage and Commercial ins.)
Home skilled visits - 6-8 per day (MINIMUM of 6 points per day at managers discretion may increase by 1 point)
1-2 Weekends per contract
Laptop, MiFi for internet access, Portable printers
BayCare Forms, BayCare Scheduling, Ace Timesheet documentation system
Other requirements:
1 year experience required
Must have valid drivers license and car to make home visits.
Must be COVID vaccinated or with exemption form on file.
Must have flu shot or declination and in both cases mandatory mask wearing at all times while at work.
Based on the needs of the HomeCare office, the visits and hours will be self-scheduled
