Position Overview

• Setting: Skilled Home Care
• Contract ID: 7251.80217.08
• Visits per Day: 6–8 home visits
• Schedule Options:
  • Monday – Friday: 8:00 AM – 5:00 PM or 9:00 AM – 6:00 PM
  • Thursday – Monday or Friday – Tuesday schedules
  • Alternating weekends required
• Scheduling Type: Self-scheduled based on HomeCare office needs

Minimum Requirements

• Experience:
  • Minimum 1 year required
  • Skilled home care experience required (private duty nursing only will NOT be accepted)
  • Home health experience with OASIS training and hands-on experience required
• Certifications:
  • BCLS required
  • COVID-19 Vaccine: Required (Medical/Religious Exemptions Only)

Clinical Skills Required

• IV therapy
• Wound care (including wound vac and advanced dressings)
• Cardiopulmonary assessments
• Foley catheter care and changes
• Staple/suture removal
• Glucometer usage
• Pro-time machine usage
• SQ/IM injections
• Lab draws (venipuncture)
• PICC line/Midline care and dressing changes
• SQ port access/de-access
• ABI compression dressing
• Pleurx/Asept drain care

Patient Population

• General medical diagnoses
• High-risk conditions including:
  • Heart Failure (HF)
  • COPD
  • Myocardial Infarction (MI)
  • Pneumonia
  • Stroke
• Post-operative care patients

Work Environment & Tools

• Skilled Home Care (Medicare, Medicare Advantage, Commercial Insurance)
• Systems Used:
  • BayCare Forms
  • BayCare Scheduling
  • Ace Timesheet documentation system
• Equipment Provided:
  • Laptop
  • MiFi (internet access)
  • Portable printer

Additional Requirements

• Locals Accepted: Yes (same rate)
• Driver’s Requirement:
  • Valid driver’s license required
  • Personal vehicle required for home visits
• Health Compliance:
  • Must be COVID vaccinated or have an approved exemption on file
  • Flu shot or formal declination required
  • Mask wearing is mandatory at all times while working
• Scrub Color: Any solid color
