LCSW – Behavioral Health / Case Management (Alaska)

Alaska (SEARHC)
Contract: 13 Weeks

Requirements:

✔ Active Alaska LCSW License (REQUIRED at submission)
✔ Flu Vaccine Required
✔ 2–4 years healthcare social work experience
✔ NABCS & ICPA Clearance required

Position Overview:

Provide mental health support, case management, and discharge planning for patients and families. Work closely with providers and community agencies to ensure access to housing, financial resources, and behavioral health services.

Key Responsibilities:

Conduct mental health evaluations & crisis counseling
Develop and manage treatment plans & care coordination
Perform psychosocial assessments & documentation (EHR)
Support discharge planning & community referrals
Collaborate with interdisciplinary teams & agencies
Provide patient & family education and advocacy
