Nurse Case Manager – Acute Care Setting (Full-Time, Day Shift)


Position Summary:
We are seeking an experienced Nurse Case Manager to join our team. This role utilizes a coordinated, collaborative, multi-disciplinary approach to assess, plan, coordinate, and evaluate the health care needs of patients throughout the continuum of care. The Nurse Case Manager serves as an advanced clinical resource to patients, families, staff, and physicians in the delivery of care.


Job Requirements:

  • Education/Experience:
  •  
    • Graduation from an accredited school of nursing.
    • Minimum 3 years of nursing experience in an acute care hospital setting.
    • At least 1 year of experience in Case Management, Discharge Planning, or Utilization Review.
    • A master’s degree in nursing with a concentration in Case Management may substitute for the experience requirement.
  • Licensing/Certification:
  •  
    • Valid RN license from the State of Nevada.
    • Basic Life Support (BLS) certification accepted by the American Heart Association (AHA).


Additional Position Requirements:

  • Minimum 3 years of Case Management experience in an acute care setting.
  • Recent documented experience in an acute care setting.
  • Pediatric Case Management experience preferred.
  • Emergency Department or Utilization Review experience a plus.
  • One or more of the following certifications/memberships are a plus:
  •  
    • Certified Case Manager (CCM)
    • Accredited Case Manager (ACM)
    • Membership in CCMC, ACCM, or ACMA


Department Review May Accept:

  • Minimum 3 years of Case Management experience in a sub-acute care setting.
  • 5 years of recent RN experience with at least 1 year as a Charge Nurse in an acute care setting.
  • Pediatric Case Management experience preferred.


Compensation & Benefits:

  • Employer-Paid Pension Plan (Nevada PERS)

  • Up to $15,000 Sign-On Bonus Eligible
  • Salary: $40.72 – $65 per hour
