Description

Position Description

The Pharmacist performs clinical and professional pharmaceutical work in the compounding and dispensing of drugs and IV supplies. The Pharmacist is responsible for reviewing prescriptions to ensure accuracy. The Pharmacist will collaborate with other healthcare professionals to plan, monitor, review and evaluate patient effectiveness and adverse effects. The pharmacist provides information and advice regarding drug interactions and potential side effects as well as analyze prescribing trends to monitor patient compliance and to prevent excessive usage or harmful interactions.

Minimum Requirements:

Current registration and/or license as a Pharmacist in state of practice required
One (1) year of experience working as a licensed pharmacist preferred
Complies with all relevant professional standards of practice
Current CPR if applicable
TB questionnaire, PPD or chest x-ray if applicable
Current Health certificate (per contract or state regulation)
Must meet all federal, state and local requirements
Must be at least 18 years of age

Schedule: 4x10 Days, Weekday and Weekends

Charting System: Cerner and Pyxis ADC

Experience Requirements:

Current Oregon Pharmacist License
Current BLS Certification through American Heart Association
1 year of Inpatient Pharmacy Experience
Previous Cerner & Pyxis Experience

Facility Details:

2.5hrs from Portland International Airport
Situated in the northwest corner of Oregon in Clatsop County, where the Columbia River meets the Pacific Ocean
25 bed critical access hospital
- average daily Med Surg census is approximately 12-15
