Job Title: Registered Nurse (RN) – Clinical Assessor
Location: Albany/Schenectady, NY
Schedule: Hybrid (2 days remote, 3 days onsite)
Pay: $55–$58 per hour

Job Overview
We are seeking a Registered Nurse (RN) – Clinical Assessor to perform comprehensive patient assessments to determine eligibility for community-based long-term care services. This hybrid role involves a combination of in-person and virtual assessments across the region. The position is focused on evaluations and documentation, with no hands-on nursing care required.

Responsibilities

• Conduct comprehensive patient assessments in both in-home and virtual settings
• Complete required clinical documentation and assessment tools accurately
• Evaluate patient needs and determine appropriate levels of care and services
• Review medical records and collaborate with care teams when needed
• Maintain compliance with state guidelines and documentation standards

Requirements

• Active Registered Nurse (RN) license in New York
• Minimum 2 years of RN experience in home care, community health, or clinical assessment
• Strong clinical assessment and documentation skills
• Ability to manage a consistent caseload and work independently
• Comfortable with hybrid work and travel for in-home assessments
