GHR Healthcare is hiring a Physical Therapist (PT) with the premiere health system in Trenton, NJ to provide home health services.

Home Health Physical Therapist – Capital Health’s LIFE Program
LIFE: Living Independence for the Elderly

Schedule: Monday – Friday, 8:00 AM – 4:30 PM
Location: Regular home visits in Mercer County & North Burlington County
No on-call duties or weekend work required

Local Pay: $68/hr-$70/hr

Traveler Pay: $2965/week

About: LIFE Program is seeking a Home Health Physical Therapist to help elderly patients maintain their independence. This role involves providing therapy services in participants’ homes, creating personalized treatment plans, and collaborating with a dedicated care team.

Key Responsibilities:

• Conduct patient evaluations and develop individualized treatment plans.
• Provide therapy to improve mobility, function, and overall quality of life.
• Educate patients and caregivers on exercises and home-based care.
• Work closely with physicians, nurses, and case managers.
• Maintain accurate and timely documentation.

Qualifications:

• Graduate of an accredited Physical Therapy program
• Licensed Physical Therapist in NJ
• AHA BLS Certification required
• Minimum 2 years of experience working with adults
• Experience in home health and senior care is essential
• Strong communication, documentation, and problem-solving skills
• Reliable transportation required

Preferred Qualifications:

• Experience in pelvic floor therapy.
• Training in Herman & Wallace Level 1A or equivalent.

Physical Demands & Work Environment:

• Frequent standing, walking, and driving.
• Occasional lifting (up to 50 lbs.), bending, and reaching.
