Description

Occupational Therapist – Home Health
Location: Charlotte, NC
Schedule: 8-hour Day Shift | 40 hours/week
 

Key Responsibilities

  • Provide occupational therapy evaluations and treatments in patients’ homes
  • Develop and implement individualized plan of care (POC)
  • Perform ADL and IADL training to improve patient independence
  • Assess need for and educate on adaptive equipment and safety strategies
  • Treat patients with chronic and post-acute conditions including:
    • Orthopedic surgeries (hip/knee replacements)
    • CHF, COPD
    • Falls, weakness, and mobility limitations
    • Occasional stroke recovery cases
  • Monitor patient progress and adjust treatment plans as needed
  • Collaborate with clinical team members to prevent hospital readmissions
  • Document care using Homecare Homebase (HCHB) system
  • Utilize technology including EPIC Rover (view-only) and Focura documentation tools

Required Qualifications

  • Active Occupational Therapist license (NC required)
  • Minimum 1 year of Home Health experience
  • BLS Certification
  • Valid Driver’s License and proof of vehicle insurance
  • Strong experience with ADLs, IADLs, and home-based therapy care
  • Experience with Homecare Homebase (HCHB) required

Additional Details

  • No on-call requirements
  • No weekend rotation
  • No major holidays
  • Average: 6 patient visits per day
  • Travel radius: ~60–80 miles daily (mileage reimbursed weekly)
  • Orientation: 5 days (NEO included)
  • Scrub color: Grey
