Description
Occupational Therapist – Home Health
Location: Charlotte, NC
Schedule: 8-hour Day Shift | 40 hours/week
Key Responsibilities
- Provide occupational therapy evaluations and treatments in patients’ homes
- Develop and implement individualized plan of care (POC)
- Perform ADL and IADL training to improve patient independence
- Assess need for and educate on adaptive equipment and safety strategies
- Treat patients with chronic and post-acute conditions including:
- Orthopedic surgeries (hip/knee replacements)
- CHF, COPD
- Falls, weakness, and mobility limitations
- Occasional stroke recovery cases
- Monitor patient progress and adjust treatment plans as needed
- Collaborate with clinical team members to prevent hospital readmissions
- Document care using Homecare Homebase (HCHB) system
- Utilize technology including EPIC Rover (view-only) and Focura documentation tools
Required Qualifications
- Active Occupational Therapist license (NC required)
- Minimum 1 year of Home Health experience
- BLS Certification
- Valid Driver’s License and proof of vehicle insurance
- Strong experience with ADLs, IADLs, and home-based therapy care
- Experience with Homecare Homebase (HCHB) required
Additional Details
- No on-call requirements
- No weekend rotation
- No major holidays
- Average: 6 patient visits per day
- Travel radius: ~60–80 miles daily (mileage reimbursed weekly)
- Orientation: 5 days (NEO included)
