Description

Position Summary

The Licensed Practical Nurse (LPN) – PRN provides direct, high-quality patient care under the supervision of a Registered Nurse (RN) or physician. In this per diem capacity, the LPN supports the nursing team on an as-needed basis, filling scheduling gaps and ensuring continuity of care. The ideal candidate is flexible, self-directed, and able to integrate seamlessly into an existing care team with minimal orientation.

 

Key Responsibilities

  • Administer medications and treatments as prescribed, following facility protocols and the five rights of medication administration.
  • Monitor and document patient vital signs, intake/output, and clinical status, reporting changes to the supervising RN or provider.
  • Perform wound care, dressing changes, catheter care, and other skilled nursing procedures within LPN scope of practice.
  • Collect specimens and assist with diagnostic procedures as directed.
  • Insert and maintain IV access; administer IV fluids and medications per state scope and facility policy.
  • Assist patients with activities of daily living (ADLs), maintaining dignity and comfort.

Documentation & Communication

  • Accurately document all assessments, interventions, and patient responses in the EHR in a timely manner.
  • Communicate clearly with the care team, patients, and families regarding care plans and changes in condition.
  • Participate in shift-change reporting and handoffs, ensuring accurate information transfer.
  • Comply with HIPAA and all confidentiality requirements.

Safety & Compliance

  • Follow infection control protocols, universal precautions, and facility policies at all times.
  • Identify and report safety hazards, near misses, and adverse events per facility protocol.
  • Maintain a safe patient environment and adhere to fall prevention and restraint policies.
  • Complete all required PRN orientation, competency validations, and mandatory annual training.

Qualifications

Required

  • Active, unencumbered Licensed Practical Nurse (LPN) license in the state of practice.
  • Graduation from an accredited LPN/LVN program.
  • Current BLS/CPR certification (AHA or equivalent).
  • Minimum 1 year of clinical LPN experience preferred; new graduates considered based on clinical rotation experience.
  • Proficiency with electronic health record (EHR) documentation.
  • Ability to work independently and adapt quickly to new environments and patient populations.
  • Reliable availability and flexibility to accept shifts on short notice.

Preferred

  • Experience in [long-term care / acute care / home health / specialty — customize as needed].
  • IV therapy certification (where applicable to state scope).
  • Bilingual (English/Spanish or other languages) is a plus.

Work Environment & Physical Requirements

  • Standing and walking for extended periods during shifts (typically 8–12 hours).
  • Lifting, repositioning, or transferring patients — up to 50 lbs with assistance.
  • Exposure to bloodborne pathogens, infectious materials, and hazardous substances; PPE is provided.
  • Fast-paced clinical environment requiring the ability to prioritize and multitask effectively.
  • Shifts may include days, evenings, nights, weekends, and holidays depending on facility need.

PRN Commitment Requirements

  • Minimum shift commitment per month as defined by facility policy (e.g., two shifts per month, including one weekend shift).
  • Must maintain current licensure, certifications, and required training to remain on the PRN roster.
  • Subject to cancellation when census is low; advance notice provided when possible.
