Job Title: Licensed Practical Nurse (LPN) – Long-Term Care / Skilled Nursing
 

Job Summary:

We are seeking an experienced and compassionate Licensed Practical Nurse (LPN) to provide high-quality nursing care in a Long-Term Care (LTC) / Skilled Nursing Facility (SNF) setting. The LPN will be responsible for administering medications, monitoring resident conditions, coordinating care, and ensuring compliance with facility policies and state regulations. The ideal candidate is detail-oriented, patient-focused, and adaptable to a fast-paced clinical environment.
 

Key Responsibilities:

• Administer medications and treatments as prescribed
• Monitor residents’ conditions and report changes to physicians and supervisors
• Record vital signs and assess deviations from normal
• Assist physicians during treatments and examinations
• Coordinate and assign nursing duties within the unit
• Maintain accurate documentation including admissions, discharges, and incident reports
• Participate in care planning, assessments, and audits
• Ensure compliance with resident rights, safety, and facility policies
• Maintain narcotics records and ensure proper handling
• Support training and orientation of nursing staff
• Maintain cleanliness, safety, and comfort of resident units

Requirements:

• Minimum 2+ years of LTC/SNF experience – REQUIRED
• Must have completed at least 1 successful travel assignment as an LPN – REQUIRED
• BLS (AHA) certification – REQUIRED

• Active Michigan LPN license – REQUIRED
