Description
Licensed Practical Nurse (LPN)
Location: Brooklyn, New York
Schedule: 40 hours per week
Multiple shifts available (Days, Evenings, Nights, Weekend-only)
Key Responsibilities
- Deliver direct nursing care to patients based on established care plans
- Monitor patient conditions and report changes to RN/physician
- Perform assessments and document patient progress accurately and timely
- Assist in developing and updating individualized care plans
- Administer medications and treatments as prescribed
- Ensure patient privacy, dignity, and comfort at all times
- Collaborate with healthcare team members (physicians, care managers, etc.)
- Support discharge planning and coordinate post-care needs
- Supervise and guide non-professional staff when required
Required Skills & Competencies
- Strong knowledge of patient care and nursing procedures
- Ability to interpret clinical data and make informed decisions
- Effective communication with patients, families, and team members
- Physical ability to stand, walk, lift, and assist patients
- Comfort working in environments with potential hazards (biological, chemical, etc.)
- Ability to operate and maintain medical equipment
Education & Experience
- Diploma from an accredited practical nursing program
- Minimum 1 year of clinical experience preferred (LTC/nursing home experience required)
Licenses & Certifications
- Active New York State LPN License
Work Schedule
- 5 days/week, 8-hour shifts
- Available shifts:
- Days: 7:00 AM – 3:15 PM
- Evenings: 3:00 PM – 11:15 PM
- Nights: 11:00 PM – 7:15 AM
- Weekend rotations required (EOW – Every Other Weekend)
Additional Requirements
- EPIC system experience preferred
- Strong professionalism required
