Key Responsibilities

• Conduct comprehensive assessments to develop individualized rehabilitation plans tailored to each resident’s needs
• Deliver expert physical therapy treatments and interventions to promote mobility, strength, and independence
• Collaborate with physicians, nurses, and other healthcare professionals to optimize patient care and outcomes
• Educate residents and their families about treatment goals and strategies to support recovery at home
• Supervise and mentor Physical Therapist Assistants to ensure high standards of care and professional growth

Required Qualifications

• Valid Physical Therapist license in Oregon
• Bachelor’s, Master’s, or Doctorate degree in Physical Therapy from an accredited program
• Current CPR certification
• At least six months of experience in a long-term care or skilled nursing facility (new graduates encouraged to apply)

Preferred Experience / Skills

• Experience working in SNF or LTC environments
• Strong communication and teamwork skills
• Passion for helping residents regain mobility and independence
