Job Title: Registered Nurse- Adult care ( Home Health)
Location: New York, NY 10004
Shift: Days
Schedule: Every other weekend is required.
Shift Length: 8 hours
Guaranteed Hours: 37.5 hours per week
Duration: 13 weeks
Start Date: 05/11/2026
Notes: This is a true at home position. Lyft rides will be available for this borough.
Position Summary:
We are seeking an experienced RN – Adult Care Home with 2+ years of overall nursing experience to join our team. The ideal candidate must currently be working in a home health setting with adult patients and have prior acute care experience in Med/Surg, ER, and Critical Care. Strong clinical skills, patient assessment, and the ability to deliver high-quality care in a home setting are essential.
Roles & Responsibilities:
- Provide comprehensive nursing care to adult patients in a home health setting
- Assess patient conditions, monitor progress, and develop individualized care plans
- Administer medications, treatments, and wound care as prescribed
- Educate patients and families on disease management and home care instructions
- Maintain accurate documentation and coordinate care with physicians and healthcare teams
Skills:
- Strong patient assessment and clinical decision-making skills
- Knowledge of home health care practices and chronic disease management
- Proficiency in medication administration, wound care, and treatment planning
- Excellent communication and patient/family education skills
- Ability to work independently and manage time effectively in a home setting
Documents Required:
- Valid NY State RN license
- ACLS Certified through AHA
- BLS Certified through AHA
- COVID card
