Job Overview:
We are seeking a compassionate and experienced Registered Nurse (RN) for Adult Care in a home care setting. The ideal candidate will provide high-quality, patient-centered care, supporting individuals in their homes through assessment, treatment, and education while collaborating with the healthcare team.
Key Responsibilities:
- Assess, plan, implement, and evaluate individualized patient care plans
- Provide direct bedside nursing care for pre- and post-surgical patients
- Administer medications and monitor for adverse reactions
- Perform wound care, IV therapy, catheter insertion, and dressing changes
- Monitor vital signs and patient conditions, reporting any changes promptly
- Utilize and adjust medical equipment as needed
- Collaborate with healthcare teams for discharge planning
- Educate patients and families on care plans and post-treatment needs
- Maintain accurate documentation and patient records
- Adhere to nursing standards, infection control protocols, and best practices
Required Qualifications:
- Minimum two (2) years of Med/Surg experience (acute or post-acute setting)
- Recent clinical experience within the past year
- Strong skills in physical assessment and patient care
- Experience in medication administration, wound care, and IV therapy
- Knowledge of post-operative care and telemetry monitoring
Certifications & Licensure:
- Active New York State RN License
- AHA BLS Certification
- AHA ACLS (Prefer)
