Registered Nurse (Case Manager)

Location: Santa Rosa, CA 95405

Contract Details - Duration: 13 weeks

Travel Experience Required: ✅ Yes

First‑time travelers accepted with clinical approval

Schedule & Staffing

Shift: Day shift – 5x8 hours

Hours: 08:00 – 16:30

Patient Ratio: 1:20

Weekends: Every other weekend, Approximately 2 weekends every 6 weeks

Requirements (Hard Stops)

• Active RN license
• Minimum 2 years acute hospital Case Management experience
  • Must include care coordination and discharge planning
  • Experience must be within the last 2 years (HARD STOP)
• Experience in a fast‑paced acute care environment
• Epic charting experience – required
• BLS certification
• Ability to work weekends as required
• Must independently arrange DME, SNF placements, and ambulance transport

 Preferred Experience

• Teaching hospital, community hospital, LTAC, or trauma center experience
• Experience with:
  • ICU, ED, MS, PCU/IMC, Pediatrics, NICU, Behavioral Health
• Familiarity with:
  • InterQual criteria
  • ICD‑10, CPT coding
  • HMO / IPA workflows

 Job Duties & Responsibilities

• Manage a caseload of approximately 20 adult and geriatric patients
• Perform comprehensive care coordination and discharge planning
• Conduct needs assessments and develop individualized plans of care
• Determine medical necessity using evidence‑based guidelines and InterQual criteria
• Coordinate DME, SNF placements, home health, hospice, and ambulance transport
• Collaborate with interdisciplinary teams including:
  • Physicians
  • Social Services
  • Physical Therapy
  • Respiratory Therapy
  • IV Teams
  • Radiology
  • Rapid Response Teams
• Organize and participate in family meetings
• Participate in development and updates of the treatment plan
• Manage prior authorizations, benefits eligibility, and payer communication
• Ensure compliance with regulatory standards including:
  • CMS
  • DRGs
  • HIPAA
  • NCQA
  • Joint Commission / National Patient Safety Goals
• Accurately document all case management activities in Epic
• Float between case management units as needed to support departmental operations
• Complete required extra checklists and audits per department policy
