CVH Care Home Health is urgently seeking a licensed Occupational Therapist (OT) for a home health assignment covering multiple areas across the East Bay. This is a flexible opportunity with two compensation models (Guaranteed vs Non-Guaranteed hours) and is open to new graduates.

Coverage Area

• Area 1: Fremont, Castro Valley, Ashland, Cherryland, Hayward, Newark, San Leandro, San Lorenzo, Union City
• Area 2: Oakland, Berkeley, Alameda, Albany, Emeryville, Piedmont
   

Compensation Model Options

Option 1: Guaranteed Hours

• Guaranteed: 36 hours/week
• Productivity Requirement: 27 points/week
• Must be flexible to travel outside assigned area or work weekends if needed
• Refusing patients voids guaranteed hours
• Not applicable during first/last week, holidays, or approved time off

Option 2: Non-Guaranteed Hours

• Paid on a per-visit basis
• No guaranteed hours
• Weekend coverage may be required
• Flexibility to travel outside service area
• Must be willing to see COVID+ / PUI patients

Productivity & Points System

• 1 Visit = 1 Point = 1.33 Hours
• Includes: Evaluations, Re-certifications, OASIS visits, Discharges, Follow-ups, etc.
• Compensation covers:
  • Travel time
  • Patient visits
  • Documentation
  • Communication (calls with patients/doctors)

Key Responsibilities

• Provide occupational therapy services in a home health setting
• Evaluate and develop individualized care plans
• Perform OASIS assessments and documentation
• Educate patients and caregivers on therapy and recovery
• Travel between patient homes within assigned territory
