Job Title: Travel RN – Case Manager (Inpatient)
Location: Baltimore, MD
Shift: Day Shift (Monday–Friday, 8:30 AM – 5:00 PM)
Duration: 16 Weeks
Job Summary
We are seeking an experienced Registered Nurse Case Manager to join a leading acute care hospital. This role focuses on care coordination, discharge planning, and utilization review for adult and geriatric patients. The ideal candidate will have strong inpatient experience and the ability to collaborate across interdisciplinary teams to ensure high-quality patient outcomes.
Key Responsibilities
- Coordinate patient care across the continuum from admission to discharge
- Develop and implement effective discharge plans
- Perform needs assessments and arrange DME (Durable Medical Equipment)
- Collaborate with physicians, nurses, and support staff
- Ensure compliance with CMS guidelines and HIPAA regulations
- Monitor patient progress and adjust care plans accordingly
- Float across Med-Surg units as needed
Requirements
- Active Maryland RN License
- BLS Certification
- Minimum 3 years of Case Management experience in an acute care setting
- Strong experience with:
- Care Coordination
- Discharge Planning
- Utilization Review
- Must have inpatient experience (Med-Surg or IMC)
