Description
Job Title: Care Manager - ED
Location: New York, NY
Duration: 8 Weeks
Shift time: Day Shift (Monday–Friday, 9:00 AM – 5:00 PM)
Education: Bachelor’s Degree (Baccalaureate Degree)
Experience:
- Minimum 5 years of clinical experience as a Registered Nurse
- Experience in Care Coordination
- Experience in Utilisation Review
- Experience in Discharge Planning
- Experience assessing patient/member clinical, psychosocial, financial, and environmental needs
- Experience creating individualised care plans with measurable goals and interventions
- Experience working with interdisciplinary care teams
- Experience managing transitions of care
- Experience addressing social determinants of health
- Experience in educating patients on chronic disease management, medication adherence, and preventive care
- Strong computer literacy and data tracking skills
- Strong critical thinking, problem-solving, and decision-making skills
- Patient-facing experience
License: Active Registered Nurse License – State of New York
Certification: PRI and Screen Certification within 3 months of employment
