Contract Details:
- Start ASAP
- 13 weeks
- Starts at 5:30p Friday, ends 8:30a Monday
- 40 hours/wk regardless of actual hours worked
- Would be responsible only for visits needed – deaths, admissions, and anyone transitioning into death – other than that, it’s whatever visits come through on-call
- EMR - Matrix Care
- 2+ yrs hospice experience required
- Must have reliable transportation to travel to patient's homes
