Role Overview
- Focus: Discharge planning (inpatient setting)
- Work closely with:
- Social Workers (shared floor responsibility)
- Physicians and interdisciplinary team
- Responsibilities:
- Patient rounds with care team
- Coordinating safe discharges
- Making referrals

Note: Utilization Review (UR) is handled by a separate department (insurance approvals, transport setup).
Patient Load
- Floor size: 33–40 patients
- Typical staffing:
- 1 Case Manager (CM) + 1 Social Worker (SW)
- Caseload:
- Standard: 17–25 patients
- Pilot floors: 12–17 patients (1 CM + 2 SWs)
- Expectation: Ability to manage 20+ patients
Clinical Areas
- Units: Med-Surg or Telemetry
- Specialties: Internal Medicine & Family Medicine
Common Duties
- Discharge planning to:
- Home
- Community programs
- Skilled Nursing Facilities (SNF)
- Additional tasks:
- Housing coordination
- Shelter packets for homeless patients
- Hemodialysis arrangements
- Wound vac referrals (KCI)
A resource binder is provided
Experience Requirements
- Minimum: 1 year inpatient case management
- Preferred:
- Critical care background
- Familiarity with:
- CarePort
- MCG
- InterQual
- Utilization Review basics
Systems
- EMR: Allscripts
Leadership
- Manager: Reiny
Schedule
- Shift: Monday–Friday, 8:00 AM – 4:30 PM
- 7.5-hour workday + 1-hour lunch
- Breaks: Per facility policy
- Culture: Encourages full lunch breaks
Weekend & Holiday Requirements
- Rare, with manager approval
- May involve:
- Covering multiple units
- Adjusted caseload
Dress Code
- Business casual OR scrubs
- Restrictions:
- No royal blue or maroon
- Neutral colors preferred
- Lab coat provided
Discharge Planning Notes
- Respiratory Therapy: N/A
- Pharmacy:
- Physicians handle medication reconciliation
- Orders sent directly to pharmacy

- 2 days hospital orientation
- Includes EMR training
